Diagnosis of Cutaneous Nocardiosis with Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry (MALDI-TOF MS)

Abstract



Nocardiosis is a rare but potentially life-threatening infectious disease caused by several species of the genus Nocardia (N.), which are aerobic, filamentous, gram-positive bacilli. A definitive diagnosis depends on the isolation and identification of Nocardia species. But identification from clinical specimens may involve performing invasive techniques on the patient and lengthy process (take up to 1 or 3 weeks) owing to slow growth, and require a professional microbiologist. Currently the genus Nocardia is best identified using 16S rRNA and 18S rRNA gene sequence analysis. Recently matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) has launched a new era in the routine microbiology laboratory. This method has proved its efficacy for the identification and diagnosis

of microorganism. MALDI-TOF MS has potential for use as a rapid (within 1 hour) and dependable method for the identification of Nocardia species with reproducibility and cost effectiveness. We report a 76-year-old woman who suffered from ulcer with papules on her right wrist and forearm. A biopsy of the skin showed granulomatous inflammation with central suppuration. A bacterial isolate from the skin was identified to be N. brasiliensis on comparative 16S rRNA gene sequencing and MALDI-TOF MS. To the best of our knowledge, this is the first case of nocardiosis in Korea caused by N. brasiliensis identified on MALDI-TOF MS.

서 론


Nocardia (N.) 균종은 그람양성 호기성 방선균으로 식물과 토양에 주로 존재하며, 인체에는 드물게 감염되나, N. asteroides, N. brasiliensis, N.otitidiscaviarum 등은 인체에 감염을 일으킬 수 있[1]. 이 중 임상적으로 가장 흔하게 동정되는 균주는 N. asteroides이며, 원발성 피부 감염에서는 N. brasiliensis가 가장 흔한 균주이다[2]. Nocardia는 면역력이 저하된 환자에서 폐, 피부 그리고 중추신경계에 감염을 일으키는 것으로 보고되며, 파종성이 강하고 화농성 육아종성 질환의 양상으로 나타난다[3].


노카르디아증의 진단은 과거 균주의 배양검사를 통해 이루어졌으나 시간이 많이 걸리며, 전문인력이 필요하였다. 이후 분자생물학적 분석이 발달함에 따라 균주의 동정이 보다 정확하고 신속하게 이루어지게 되었으며, 16S 리보솜 RNA 염기서열 분석이 흔히 사용되고 있다[2]. 최근에는 합성 고분자의 분자량 분석이나 Bio 관련 커다란 분자 (주로 단백질, 탄수화물)의 분자량 분석에 사용되는 matrix-assisted laser desorption ionizationtime of flight mass spectrometry (MALDI-TOF MS) 를 이용한 신속하고, 정확한 균주의 동정이 다수 보고되고 있다[4].


면역력이 저하된 환자들에게 주로 감염되는 노카르디아증은 초기 진단이 중요하다. 이에 저자들은 균배양 및 16S 리보솜 RNA 염기서열 분석뿐 아니라 이전 국내에 보고된 바 없는 MALDI-TOF MS를 이용하여 노카르디아증을 진단하고 이의 유용성을 문헌 고찰과 함께 보고한다.



증 례


환 자: 배OO, 76세, 여자


주 소: 오른팔의 통증을 동반한 경계가 불명확한 홍반성 궤양


현병력: 환자는 내원 3주 전부터 오른 손목에 통증을 동반한 구진이 발생하여 점차 병변의 크기가 증가하며 부종 및 분비물을 동반하는 궤양으로 변화하였다.


과거력 및 가족력: 수년 전부터 고혈압과 류마티스 관절염으로 methylprednisolone, methotrexate 을 포함한 약제를 장기복용 중이었으며, 외상 병력은 없었다.


피부 소견: 오른 손목 부위에 다양한 크기의 경계가 불명확한 홍반성 궤양과 병변 내부 및 주위에 다양한 크기의 홍반성 구진이 관찰되었다(Fig. 1).


이학적 소견: 피부 소견 외 특기사항 없음 병리조직학적 소견: 표피는 불규칙적인 가시세포증을 보였고, 진피에 다수의 호중구와 림프구를 동반한 화농성 염증 소견이 보였다 (Fig. 2A, 2B). D-periodic acid schiff (PAS)와 Gomori-methenamine silver (GMS) 염색에서는 음성 소견을 보였다 (Fig. 2C, 2D).

진균학적 소견: 생검 조직을 혈액 한천배지와 3% Ogawa 배지에 접종하여 37℃에서 배양한 결과 하얗고 건조한 양상의 균집락을 관찰할 수 있었다 (Fig. 3A, 3B). 배양된 집락의 도말염색 결과 가늘게 분지된 그람양성 간균이 관찰되었으며, 항산성 염색에서는 약양성 소견이 관찰되었다 (Fig.3C, 3D).


분자생물학적 검사: 잘 형성된 집락으로부터 유전자를 추출하여 중합효소 연쇄반응 {Polymerase Chain Reaction (PCR)}을 이용한 16S 리보솜 RNA 염기서열 분석을 시행하였고, 분석된 결과를 Basic Local Alignment Search Tool을 이용하여 NCBI GenBank의 염기서열과 대조한 결과 N. brasiliensis (GenBank accession number NR_074743.1) ATCC 700358 strain HUJEG-1과 99% 일치하는 소견을 얻었다.


MALDI-TOF MS: Microflex LT (Bruker Daltonics, Leipzig, Germany)와 MALDI-TOF BioTyper (Bruker Daltonics, Leipzig, Germany)를 이용하여 동정하였(Fig. 4). 대략적인 방법은 다음과 같다. 배지에서 자란 집락을 metal plat에 직접 도말하여 건조시키고, 회사에서 제공한 매트릭스 용액을 첨가하여 다시 건조시킨 후 Microflex LT 장비에 장착 하였다. 장비의 검출기에 의한 결과는 MALDITOF BioTyper program (version 2.0)에 의하여 표준과 비교하여 동정되었다. 동정치 (score value)의결과 해석은 기기회사의 기준에 따라 2.0 이상이면 균종동정이, 1.7~2.0인 경우 균속동정이 가능한 것으로, 1.7 이하이면 신뢰성이 떨어지는 것으로 판정하였다. 동정결과 배양 및 분자생물학적 검사결과와 일치하게 N. brasiliensis로 확인되었다 (Fig. 5).


치료 및 경과: 초기에 ampibactam 3 g을 1일 3회 경구투여와 mupirocin 연고 도포로 치료하였으나 병변의 호전 보이지 않았고, 이후 trimethoprimsulfamethoxazole (TMP/SMX) 80/400 mg을 1일 2회 경구투여로 4주간 치료 후 피부 병변은 호전되었다.

고 찰


Nocardia는 1888년 Edmond Nocard가 소에서 최초로 발견, 분리하였으며, 1890년 Eppinger에 의해 인체에 감염된 임상 증례가 처음 보고된 호산성, 비운동성의 그람양성 균주로 어디에나 존재하고 특히 토양에 널리 분포하고 있다[5]. 처음에는 진균학 분야로 구분되었으나, 현재는 진균양 세균 (fungus-like bacteria)으로 분류되며, 이는 기질의 직경이 1 μm 이하, PAS 염색에 음성, sulfonamide 등 의 항진균성 작용이 없는 항생제에 대한 감수성,세포막에 sterol이 없는 특성 등을 가지고 있기 때문이다[6].

Nocardia는 공기 중의 균의 흡입에 의한 경우와, 드물지만 외상 후 직접 피부를 통해 주입되는 경우가 있다. 균의 흡입에 의한 폐 감염이 가장 흔한 형태이며, 이 중 대부분은 N. asteroides에 의해 발생한다[7]. 원발성 피부 감염증은 피부의 상처를 통한 Nocardia의 주입에 의해 발생하는 드문 질환으로, 이 경우 N. brasiliensis와 N. transvalensis 등이 주된 원인이다[1]. 노카르디아는 흔하지 않지만 생명을 위협할 수 있는 감염성 질환으로 최근 노카르디아증에 대한 보고가 증가하고 있다. 전형적으로 림프구성암, 쿠싱증후군, 후천성면역결핍증후군, 장기이식 수혜자, 고용량 스테로이드 복용 환자 등과 같이 면역억제 상태에서 발생하며 세포성 면역억제가 노카르디아 감염 발생에 중요한 역할을 한다고 알려져 있다[8]. 본 증례의 경우 다른 피부노카르디아증의 국내보고에서와 유사하게 고령의 환자이며 기저 질환이 있었다. 현재는 16S rNRA 유전자 염기서열 등과 같은 새로운 분 자기법 등을 통해 50여종의 Nocardia가 확인되었고, 이 중 N. asteroides, N. brasiliensis, N. pseudobrasiliensis 등과 같은 16여 종의 인체 감염이 보고된 바 있다[9],[10].

노카르디아증은 sulfonamide가 치료 약물로 사용되어 왔으며, 특히 trimethoprim과 sulfamethoxazole의 병합요법이 좋은 효과를 보인다. 그런데, 균 동정이 빠르고 정확하게 이루어지면 치료를 보다 효과적으로 시행할 수 있으므로 균을 동정하는 많은 방법들이 발전하고 있다. 노카르디아증의 진단에는 배양검사, 도말염색 및 변형 항산성 염색이 이용되며, brain heart infusion 한천배지나, Sabouraud dextrose 우무배지, 혈액우무배지 등 일반배지에서 잘 배양된다[2]. 배양 소견은 하얗고 주름진 건조한 양상의 집락을 보이며, 서서히 자라서 균종의 종류에 따라 3일에서 2주 이상의 배양기간이 필요하다. 도말염색에서는 그람양성 막대균으로 관찰되며, 산의 농도를 낮춘 변형 항산성 염색에서는 양성을 보인다. 균주를 동정하기 위한 방법으로 기존에는 urea hydrolysis, pyrrolidonyl aminopeptidase, gamma-glutamyl aminopeptidase, alpha-mannosidase, alpha glucosidase 등의 생화학적 방법을 사용했지만 시간이 많이 걸린다는 단점이 있다[3]. 이러한 단점을 보완하기 위해 Wauters 등[11]은 임상적으로 흔히 관찰되는 6종의 Nocardia를 효소적 방법 (enzymatic test)을 이용하여 신속하게 진단하는 방법을 소개한 바 있었으며, Conville [12]이 중합효소 연쇄반응을 이용한 16S rRNA 염기서열 분석을 시행한 이후 현재까지 신속하고 정확한 방법으로 이용되고 있다.


MALDI-TOF는 검체를 이온화하여 진공관에서 검출기에 도달하는 시간을 근거로 구성 물질의 질량을 측정하는 방법이다. 최근 개발된 MALDI Biotyper는 MALDI-TOF MS 기법을 이용하여 얻은 세균의 단백질 성분을 분석하여 균을 동정하는 프로테오믹 기법 (proteomic method)으로, 이미 구축된 각 균종의 정보와 비교 분석하여 다양한 미생물 (microorganisms)을 감별, 동정하는 미생물학에서의 새로운 분야이다[13]. MALDI-TOF MS는 분석체 (analyte)의 조각으로부터 원래 분석체의 질량을 예측하는 기존의 질량 분광학과는 달리, 시료에 자외선을 흡수하는 매트릭스 (결정화가 가능한 저분자 유기화합물)를 첨가하여 결정화시킨후, 레이저를 조사하여 이온화시켜 생성된 이온들의 비행시간의 차이로 질량을 분석하는 분석법이다[14]. 이런 방식으로 최근 그람양성 세균, 그람음성 세균, 장내세균, 효모균 등 다양한 microorganism을 분석하는 보고가 나오고 있으며, 기존의 방법으로는 동정하기 까다로운 Nocardia 같은 organism의 동정시간을 줄이며, 정확하고, 쉬우며, 상대적으로 경제적인 MALDI-TOF MS로 동정하는 보고가 나오고 있다[4],[15]. 이전의 보고에 의하면 MALDI-TOF MS 동정시간은 배양균주당 평균6분이며, 산출된 동정 비용은 상품화 키트를 포함한 전통적 동정 방법의 22~32%로 임상 미생물 검사실에서 이용하기에 간편하고 동정에 소요되는 시간이 매우 짧으며 소모 비용이 적다고 보고하고 있다[16],[17],[18]. 또한, MALDI-TOF MS를 이용한 산소성 세균의 동정은 생화학적 성상을 기반으로 하는 전통적 방법, 상품화 kit와 높은 일치율을 보였다는 보고가 있다[13]. 하지만 동정치의 신뢰도가 낮은 경우는 직접도말법 재검에 단백추출법을 추가하여 대부분 균종동정 결과를 얻을 수 있다고 보고되고 있으나 직접도말법 재검 및 단백추 출법 추가로도 동정치의 신뢰도가 낮은 소수의 균주들은 전통적인 동정법과 분자유전학적 방법을 추가할 필요가 있을 것이며, 구축된 세균의 단백질 정보의 부족으로 인한 제한 등이 발생할 수 있을 것으로 생각된다.


본 증례에서는 검체를 혈액우무배지와 3% Ogawa 배지에 배양한 결과 하얗고 건조한 균집락을 관찰할 수 있었고, 배양된 집락의 도말염색 결과 가늘게 분지된 그람양성 막대균이, 항산성 염색에서는 약양성 소견이 관찰되어 노카르디아증으로 진단되었다. 또한 분자생물학적 검사에서도 N. brasiliensis로 진단되었다. MALDI-TOF MS를 통해서도 N.brasiliensis를 확인할 수 있어 이 방법이 노카르디아증의 진단 및 균종 확인에 유용한 방법이 될 수 있음을 확인하였다. 이에 저자들은 노카르디아증이 의심되는 경우에 빠른 진단을 위해 MALDI-TOF MS를 이용할 수 있음을 알리고자 이 증례를 문헌 고찰과 함께 보고한다.
